HHT Care Checklists

The checklists are for people with HHT to self-manage their care and bring to their doctor. These checklists are based on the care recommended in the HHT Guidelines.

Start with the "My HHT Care Checklist" and then add all of the other Checklists that might apply (even if you are not sure), to your HHT Care personal folder. Review the checklist items with your doctor and also the Guidelines recommendations that follow each checklist.

My HHT Care Checklist

The “My HHT” checklists are a series of checklists by topic area for people with HHT to self-manage their care and bring to their doctor. These checklists are based on the care recommended in the HHT Guidelines.
